Completing the necessary forms before arriving to your appointment will help to make your visit faster. Please print, complete, and bring the form(s) with you to your appointment.
Health History Form - This is for all students who will be making their first visit to Health Services (this is the same form you received in your Orientation packet), and any student who has had a significant change in their health status since the last time they filled out this form.
Female History/GYN Form - This is for all women coming in for gynecological check-ups/ annual GYN appointments/ pap smears.
Note: If you have not downloaded and completed the form(s) listed above, please arrive to your appointment 10-15 minutes early. This will ensure enough time for you to complete the form at Health Services. If you are more than 10 minutes late to your appointment, you will have to reschedule.
When you arrive to Health & Wellness Services, please make sure to check in with a staff member at the front desk and tell them why you are here.
Make sure that you have your health insurance card and bring it with your to your appointment.
Because walk-in hours fill up quickly, it is best to call Health & Wellness Services to see if all slots are filled. If there are slots available, it is best to arrive as close to the beginning of each walk-in session as possible (i.e. if walk-in hours start at 10 am, you should arrive at 9:45 am).
Note: Urgent Care Walk-In hours are for urgent matters. A good rule for an appropriate Urgent Care Walk-In visit is that you are feeling ill enough to miss class or work or have a minor injury (i.e. sprains, cuts etc). Urgent Care Walk-In hours can also be used for time sensitive issues such as: you need emergency contraception or you are currently bleeding.

All visits to Health & Wellness Services and conversations with the staff are confidential. Your permission must be obtained before we will give information to anyone not directly related to your care. This includes parents and University officials.
